If you are submitting a CD or DVD job, please follow the steps below to ensure that you have everything you need to get your job started!
- CD or DVD Master
Click here to download our Master Specifications Sheet. This will ensure that your master disc is formatted correctly for duplication or replication.
Need us to create a master disc for you? Submit your files to us and we can create a master CD or DVD for you!

- Artwork files
Download the appropriate CD/DVD face label template to make sure your artwork is designed with the correct specifications. This will ensure that your artwork has the optimum output for duplication or replication.
Need help getting artwork created? No problem! Let us know what you have in mind and we can design your artwork for you!
- Choose your disc packaging

- Upload your files to our FTP site
Once you have your artwork files ready to go, you can login via our FTP site. Request a password from your account representative then create a folder with your job name and simply drag and drop your files in. If Disc, Inc. is creating your master, you can submit your master files the same way.
You can also bring in your master disc and artwork files to our location. We would love to see you!
- Contact your Disc, Inc. representative
Email or call your Disc, Inc. representative and let them know that your files have been uploaded.
Don’t have a representative? No problem. Just call 800-580-3472 and ask to speak to a sales associate.

Credit Application Form
If you would like to set up an account with Disc, Inc. please download our credit application form. Once it is complete, fax it to your account representative at 713.864.7849.
IPR Form
If your master contains copyrighted or licensed material please click here, download and complete the appropriate form and fax to Attention: CSR @ 713.864.7849.
Duplication / Printing Rights Form
If Disc, Inc. has created your artwork and/or master please click here, download and complete the appropriate form to approve or waive your rights to approve. Once completed fax to Attention: CSR @ 713.864.7849.
Hard Drive Recovery Form
If you are bringing your drive in for data recovery, download the required forms here and submit them with your drive. Please note that these forms are required for all recovery jobs and diagnostics will not begin until they have been received.
Credit Card Authorization Form
If you would like to charge your project to your credit card, please download our Credit Card Authorization form. When complete, fax it to your account representative at 713.864.7849.
Texas Resale Certificate
If you are going to be reselling the products produced by Disc, Inc. please click here, download and complete the appropriate form and fax to Attention: Accounts Payable @ 713.864.7849.
Texas Sales and Use Tax Exemption Certification
